At Moonshadow Therapeutic Massage, we
realize that sometimes filling out forms in the waiting
area can be stressful in itself. For your convenience, we
have included our intake form below for you to print and
fill out at home or at your office. But please be sure to
bring it along and hand it to the therapist before your
first session. Otherwise, you will need to arrive 15 minutes
early to fill one out in our office. Please note that you
will need Adobe Acrobat Reader to open the file. You can
